Position Snapshot

• Location: Ellsworth, Kansas (Must own or rent your current home)
• Supporting: We have a male client that enjoys going for walks and is looking for an older male provider who shares his love for spending time in the garage or shop. His perfect match is someone who engages but also gives him space when he needs it. He would benefit from a home that’s calm and quiet, where all family members are at least close to adult age.  
• Average Annual Income: Approximately $47,000 (federally tax-free) + Monthly Rent & Household Expenses Contribution
• Work from Home Opportunity
• Ongoing Training & Team Support

How You’ll Make a Difference

Every individual is unique, but your responsibilities may include:

• Supporting daily living skills such as cooking, budgeting, cleaning, and decision-making.
• Assisting with medication administration and medical appointments.
• Helping someone build connections, participate in their community, and experience a sense of belonging.
• Providing transportation and support with daily activities as needed.
• Maintaining documentation and communication with the Mosaic support team.
• Helping someone build confidence, develop independence, and pursue personal goals.

Depending on the individual’s needs, support may also include:

• Assistance with personal care activities
• Behavioral support
• Medical or physical support needs
• Specialized care and monitoring

Requirements

• Must be at least 18 years of age
• You must be a current resident of the state of Kansas (due to state funding restrictions).
• Own/Rent your own home, apartment, condo, or townhouse
• Active homeowner’s or renter’s insurance
• Reliable transportation, valid driver’s license, and current vehicle insurance
• Internet access and basic computer skills
• Ability to complete required screenings, background checks, and training

Experience is preferred but not required.
